About the Form 1095-C

What should I do with Form 1095-C?

What other forms may I receive? And how are they different?

Why did I receive more than one Form 1095-C?

What information is included on the Form 1095-C?

There are three parts to the form:

  • Part I – Employee and Employer Information 报告你和你的雇主——马萨诸塞州联邦的信息.
